To ensure you have read and understood the requirements needed to take part in this Work Experience Arrangement, please complete the form provided within 7 days of receiving the email.

About the program:

The workplace learning program is beneficial to both students and host employers. Benefits to industry include:

– Giving employers a valued training role in assisting young people in acquiring workplace skills

– Providing access to a pool of pre-skilled and immediately productive young workers from which employers can recruit

– Providing a direct link to schools and the opportunity to make education more relevant to industry needs

It is important to note that the students may not receive any form of compensation for services rendered as they are not covered by Workers Compensation. Students are covered through the College insurance policy.

It is a requirement that students should sign in and out every working day and that their logbooks are signed off by their assigned supervisor. Should they leave the premises earlier due to illness or any other unforeseen circumstance, it is very important to get written permission from either the parent or myself before they can be released. It is understood that they will be under supervision for all activities.

It is the student’s responsibility to inform you if they are unable to attend work. If the student does not arrive at work, please inform the Workplace Learning Coordinator at the College on 9314 7722.

During their placement, students will be visited by a teacher from Kennedy Baptist College. This visitation will be organised at a time that is convenient for you.

Students will receive an official report at the end of the Work Experience. This could be of assistance with future job applications.

I agree to the privacy policy.

I, consent to my taking part in this Work Experience Arrangement and I:

1. understand occupational health and safety legislation and standards relevant to the conduct of my undertaking and will comply with these laws and standards with respect to the student as if the student were my employee.

2. will identify all hazards relevant to the conduct of my undertaking and will assess and control all related risks. Please stipulate what identifiable risks cannot be controlled in the Occupational Health Safety section.

3. have read and understood the Work Experience Guidelines for Employers. I will ensure that required planning, induction, supervision and safe systems of work are always provided for the student to maintain a safe and healthy Work Experience Arrangement.

4. will consider the competency, maturity and physical capabilities of the student in relation to all activities he or she will undertake. The Student’s program of activities will be planned and carried out with these considerations in mind.

5. will nominate a Supervisor (or Supervisors) of the student who will be responsible for ensuring that my obligations as the student’s Employer are carried out.

6. will provide appropriate information, training, instruction and supervision to the student in respect of occupational health and safety and will provide any equipment and/or clothing which is required to comply with my duty of care toward the student.

7. will ensure that the Work Experience is undertaken in a non-discriminatory and harassment-free environment.

8. will permit access to the workplace and contact with the student by the Principal, the Work Experience Coordinator, or a teacher at any reasonable time during the Work Experience Arrangement.

9. will ensure that the Work Experience Arrangement is not used as a substitute for the employment of employees or the engagement of contractors and the payment of appropriate wages or fee for services to employees or contractors respectively.

10. confirm that where the Work Experience Coordinator has disclosed any necessary health information in relation to the student. I will maintain the confidentiality of that health information and only disclose this information to another party if treatment is required for a known medical condition or in the case of a medical emergency.
